Please review our Office Policies as stated below:

  • CoPays are decided upon by your insurance company and are due at the start of each visit.  We accept cash, check, credit and debit cards.
  • If you are running late for your scheduled appointment, please call the office.  Anyone more than 15 minutes late, may have to reschedule their appointment time so as not to interfere with the appointments of others.
  • If at all possible, forms requiring medical information such as physical forms for school, daycare, jobs or athletics should be filled out at the time of the physical.  When this is not possible, there is a $5 fee for filling out forms and they must be dropped off, paid for, and picked up no sooner that 24 hours later.
  • Driver (Learner's Permit) forms must be signed in front of the doctor and must have the front side filled out.
  • There is a $5 fee for any letters that the Office Manager writes for schools, lawyers, etc.This fee must be paid prior to the letter being sent.
  • Patients who are 18 years of age have the right to manage their own care and must fill out a Patient Consent Form telling us who has permission to receive medical information about the patient.
  • School Excuses are given at the time of the appointment and the parent/caregiver is responsible for getting it to the school.  School excuses may be written for children staying home but not seeing the doctor ONLY if we are notified on the day of the absence.  No excuses will be written for days when we were not called. After 3 days of absence, the child/children must be seen in the office. All excuses for absences not accompanied by an office visit are faxed to the school on Friday.  If the excuse is needed sooner, the parent/caregiver must pick it up at the office.
  • Letters to Utility Companies are only written to your power company and only for patients using life-sustaining medical equipment, such as monitors or oxygen.
